Important information to bring to your tax appointment

Anything marked “IMPORTANT TAX DOCUMENT”:

  • W-2 (wage income)
  • W-2G (gambling winnings)
  • 1098 (mortgage interest statement)
  • 1098-C (vehicle donations)
  • *NEW* 1098-E (student loan interest)
  • 1098-T (tuition) 1099-B (capital gains transactions)
  • 1099-INT (interest income)
  • 1099-DIV (dividend income)
  • 1099-G (state tax refunds and unemployment)
  • 1099-MISC (rents, self-employment & other income)
  • 1099-R (distributions from 401(k)s, IRAs, pensions and annuities)
  • 1099-S (real estate sales)
  • 1099-SSA (Social Security)
  • K-1 (income from trusts, partnerships and S-Corps.)
  • *NEW* Amounts paid for home heating fuel (natural gas, oil, propane) for January through March 2006. This is a new Mass. deduction for certain taxpayers earning less than $75,000.
  • *NEW* Amounts paid for energy-efficient heating items. This includes home insulation; new window installation; advanced programmable thermostats; fuel efficient furnaces, boilers, oil, gas, propane or electric systems; solar domestic hot water systems; materials for insulation or sealing of a duct, attic, basement, rim joint; and wall and pipe insulation for heating systems. This is a new Mass. credit (for items purchased between November 2005 and March 2006) and Federal credit (for items purchased in 2006) that may be worth as much as $1500 for certain taxpayers.
  • *Back for 2006!* Amounts paid for MBTA passes (including subway, bus and commuter rail) and Fastlane tolls paid via transponder.
  • *Modified for 2006!* Amounts paid for qualified hybrid vehicles. See the complete list of qualified vehicles by clicking here. The credit could save you from $250 to $2600 in Federal taxes. Your dealer should provide you with applicable tax credit information.
  • *NEW* Amounts paid for telephone excise tax on long distance services from February 2003 through July 2006. Note that for individuals, if you can not establish the amounts paid for those years, the IRS has a standard amount ($30 – $60) we can use to qualify for this one-time credit. Also, tell your friends and family who are not required to file a return due their income that they can file a return solely to claim the credit. Note that if you are self-employed or own a corporation or partnership, you must use the actual amount of tax paid unless the IRS later prescribes standard credit amounts.
  • Social Security card and date of birth for dependents.
  • Rent paid while a resident of Mass.
  • Amounts paid to repair or replace a failed septic system or to remove lead paint from your home.
  • Amount of alimony paid or received and Social Security Number of former spouse.
  • Closing paperwork for new home purchases or old home sales.
  • If you sold mutual fund shares, stocks, bonds or other investments: provide the date(s) you purchased the investments and the original amount(s) paid.
  • Amounts paid for student loan interest and/or higher education expenses.
  • Amount of IRA contributions you made or plan to make for tax year 2006.
  • If you paid for childcare in order to work or attend college: provide the name and address of the recipient; the amount paid for 2006; and the recipient’s taxpayer identification number.
  • If you made estimated tax payments, provide the amounts & dates paid.
  • **NEW** Bank account information or blank check if you wish to have a refund directly deposited to up to 3 accounts (this includes, checking, savings, IRAs, etc.). Also, if you have a Mass. or IRS balance due, you can have the amount debited from your account on the date you choose.
  • If you suffered a casualty loss (flood, fire, hurricane, etc.) bring proof of loss, insurance papers, etc to determine if you have a tax deductible loss.
  • ALL NEW CLIENTS should bring their past 3 tax returns (if available) for review.
